HCRG Care Group is seeking a flexible Bank Health Visitor for their services in East Lancashire, specifically in Burnley, Pendle, Ribble Valley, and Hyndburn & Rossendale. This role allows for weekend work and additional hours, ideal for those who cannot commit to a typical weekday schedule.


Key Responsibilities:

  • Deliver the Healthy Child Programme, assessing health needs and evaluating care packages.
  • Utilize the Family Health Needs Assessment tool to formulate and document care plans.
  • Create individualized care plans for children aged 0-5, engaging families in the care process.
  • Identify health and development needs, particularly for the 0-5 age group.
  • Collaborate with other agencies, leading where appropriate, to ensure effective partnership working.
  • Maintain high standards in public health nursing through evidence-based practices.
