CLINICAL FELLOW ST3+ NEUROSURGERY

Applications are invited for the post of Clinical Fellows in Neurosurgery to start as soon as possible, at Southmead Hospital which is part of North Bristol NHS Trust. The post holder(s) will work in the award winning Brunel building at Southmead Bristol and will benefit from the large clinical caseload catering to an adult population over 2.5 million and delivers in excess of 4000 emergency and elective operations a year. All Consultant neurosurgeons have a sub-specialty interest and all the major neurosurgical subspecialties such as Oncology, skullbase, vascular, epilepsy, movement disorders, complex spine and complex pain are well represented. There are 64 ward beds, 12 neuro HDU beds as well as additional shared ITU beds and a wide range of allied clinical specialties such as neurology, neuropsychiatry, neuropsychology, Speech & language therapy and specialist neuro-rehabilitation. The successful applicant will be expected to have adequate experience in neurosurgery to take part in the emergency work of the department (multi tier middle grade rota) including competency in basic cranial and spinal procedures. The successful applicant will be assigned to work with one of the above Consultants. They will provide assistance in theatre, undertake a daily ward round and liaise with their Consultant regarding management decisions. They will attend outpatients and the clinical meetings. There is a daily 8:00 a.m. combined clinical and radiology meeting; a weekly tumour MDT, fortnightly skullbase MDT, weekly epilepsy and movement disorders MDT, regular journal clubs and an academic teaching program. The successful applicant will have the opportunity to participate in departmental teaching and would be expected to participate in clinical governance and audit activities.

The out of hours commitment is a hybrid rota (consisting of 18 doctors) of four tiers of middle grade doctors depending on their experience in neurosurgery with the senior most tier providing support as non resident on-call. The emergency neurosurgical work will be from the same catchment population. There is a separate rota of complex spine surgery consultants (drawn from both neurosurgical and orthopaedic spinal units) who support the on-call registrar.

• Operative experience in basic neurosurgical procedures required for on-call service
• Neurosurgical Registrar level experience
• MRCS/FRCS or equivalent.

• The successful applicant will be required to provide documentary evidence of natural or acquired immunity to hepatitis B and other transmissible infections as required by Occupational health. Where this is not possible, the postholder will be required to demonstrate by recent (within the last year) evidence of serology showing immunity. These provisions are to meet the requirements of the Department of Healths instructions to trusts (HSC(93)40).

• The full-time equivalent basic salary applicable on commencement will be assessed on the basis of your work schedule and may comprise one or more of the following:

• If your work schedule requires you to undertake additional hours of work over and above the standard week of 40 hours, you will be paid at the rate of 1/40th of the full time equivalent basic pay;

• If part of the work in your rota is undertaken at a time which attracts an enhanced hourly rate of pay, an enhancement of the hourly basic pay rate shall be paid on any hours worked between 21.00 and 07.00, on any day of the week. The number of hours in the rota for which an enhancement is paid will be assessed across the length of the rota cycle

• If you are required to participate in work at the weekend, at a minimum frequency of 1 in 8 across the length of the rota cycle, you will be paid an allowance.

• If you are required to participate in an on-call rota, you will receive an on-call availability allowance
• Exception reporting is the mechanism used by doctors to inform the employer when their day-to-day work varies significantly. These variations can be
• Differences in the total hours of work (including opportunities for rest breaks)
• Differences in the pattern of hours worked
• Differences in the educational opportunities and support available to the doctor
• Differences in the support available to the doctor during service

• Exception reports allow the employer the opportunity to address issues as they arise, and to make timely adjustments as necessary.:
• The minimum notice to be given by you, in writing, to terminate your employment is 3 months. Please refer to Section 8 of the North Bristol NHS Trust Terms & Conditions of Service for Trust Clinical

• Your fixed term end date has been given at the outset (see page 1) thereafter no further notice will be given. However, the Trust reserves the right to end a fixed term contract prior to the stated end date, if required. In this situation you will be given three months notice and the reason for the cessation of the contract will be explained to you.

• Should you be dismissed under the Trusts Disciplinary Procedure for Gross Misconduct the Trust reserves the right to dismiss you without notice.

• The Trust also reserves the right to make payments in lieu of notice where it deems this to be
• You are required to be registered with the General Medical Council, as indicated on your job description, throughout the duration of your employment.

• You are normally covered by the NHS Hospital and Community Health Services indemnity against claims of medical negligence. However, in certain circumstances (especially in services for which you receive a separate fee) you may not be covered by the indemnity. The Health Departments therefore advise that you maintain membership of your medical defence organisation
• Where a doctor intends to undertake hours of paid locum work, additional to contracted hours, the doctor must initially offer such additional work exclusively to the NHS (this is not restricted to the hospital that the Doctor is working at).
• All medical practitioners taking up an appointment involving the treatment of children must have their Heaf test and general immunity state checked. This may be arranged by contacting the Occupational Health Department in the Trust, where all vaccinations and immunisations may be obtained. All other staff are advised to similarly have their general immunity state regularly checked.

• The passing of a medical examination is a condition of appointment to medical and dental posts in the hospital service.

• You are required to provide documentary evidence of natural or acquired immunity to hepatitis B. Where this is not possible, the postholder will be required to demonstrate by recent (within the last year) evidence of serology showing the absence of hepatitis B surface antigen. These provisions are to meet the requirements of the Department of Health's instructions to Trusts (HSG(93)40).
• Should you be overpaid salary or expenses, you agree that the Trust recover such sums from your salary and precise overpayment and repayment arrangements will be discussed with you
