Band 4: £26,928 to £29,551 a year per annum  PERMANENT 

As an Assistant Practitioner Nursing the post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a registered practitioner with appropriate supervision in place. Understanding evidence-based practice and delivering care in line with current evidence, the post holder will take responsibility for the provision of person-centred care in accordance with organisational policy and procedures. Within the role the post holder will:

o Be responsible for some elements of patient assessment, undertaken in conjunction with the registrant, implementing programmes of care, and modifying programmes of care as delegated by a registered practitioner. The assistant practitioner will be allocated a group of patients to care for, in the same way that patients are allocated to a registered nurse. The supervising registrant will undertake elements of care that sits outside of the band 4 scope of practice.

o Report regularly to the registered practitioner, recognising own limitations and seeking advice if they are unsure.

o Work in support of the multi-disciplinary team: registered nurses, doctors, and allied health professionals to provide a seamless service to patients with proximal supervision from a registered nurse/practitioner.

The team of assistant practitioners is vital to providing equitable access to care across the whole of the region in some of the most deprived and disconnected areas of Swansea Bay, where often those most at risk of being affected by viral hepatitis have the least access to health promotion, harm reduction, and specialist intervention. This team will bridge the gap between those in the community and the expertise of the clinical nurse specialists leading elimination efforts by 2.

The team will work effectively alongside not only the BBV practitioners but also the wider multidisciplinary team including Integrated Sexual Health, Respiratory, and Homeless Health to maximise the potential of each interaction and deliver resource-efficient and cost-effective screening and an effective pathway to specialist care.

Education and elimination projects undertaken by the team of assistant practitioners will involve locally innovated and

evidence-based high intensity test and treat methods, delivering high-efficiency clinical interventions in a cost-effective and resource-prudent manner, tackling the hidden burden of infection and the complex social stigma associated with addressing this.
