MAIN DUTIES AND RESPONSIBILITIES

- To assess patients needs and develop, implement and evaluate programmes of care.

- To organise the delivery of nursing care, deploying staff effectively.

- To be aware of, and communicate to team members, changes to patient information and treatment plans, both verbally and in writing.

- To adhere to NMC standards of record keeping.

- To assist the Ward Sister in setting, monitoring and maintaining standards of care.

- To participate in nursing audit and service improvement activities eg Essence of Care

- To be aware of resource constraints and the importance of working within budget.

- To provide advice and act as role model to other members of the team.

- To be proactive in managing risk and maintaining patient safety.

- To supervise the work of nursing support staff, ensuring tasks undertaken have been delegated appropriately.

- To contribute to the day-to-day management of the Directorate, co-operating with colleagues to provide an appropriate level of nursing service and to address bed management issues.- To participate in education and training of junior staff.

- To involve patients/carers in the planning and delivery of care.

- To participate in clinical supervision.

- To assist in resolving conflict and defusing situations where appropriate.

- To promote a positive and professional image of nursing at all times.

An exciting opportunity has arisen for a full time band 5 Nurse to join the Stroke Team at University Hospital of North Durham. We are looking to recruit an enthusiastic and motivated person with an interest in Stroke to be part of our expanding team in delivering high standards of care to all our patients.

The stroke unit at UHND is a 32 bedded unit with 8 dedicated to hyper acute stroke. Our aim is to provide a holistic care for acute stroke patients through a multidisciplinary approach by working with our stroke specialist teams of therapists, doctors and nurses. The unit receives direct stroke admissions from the paramedics, accident and emergency unit, emergency assessment unit and gp practices. One of the integral part of our stroke care is the provision of thrombolysis for eligible patients 24 hours a day and referrals to regional thrombectomy centers 7 days a week. The Stroke multidisciplinary team is passionate about getting it right for patients and their families.

The post holder will be reposonsible for delivering a high standard of care to patients and their loved ones. They will organise and plan care, depending upon the patients needs ensuring all care needs are met. They will be accountable for their care and supported by colleagues.
