As a Healthcare Support Worker with Sound PCN you’ll be part of a home‑visiting team delivering same‑day, community care across five practices in north‑west Plymouth. You’ll visit patients in their homes and care homes, with a focus on frailty and older people, carrying out baseline observations (pulse oximetry, blood pressure, temperature and pulse), routine BP monitoring, wound care and dressings, BMI checks and specimen handling. You will support chronic disease clinics, assist practice nurses with health promotion, act as a chaperone and—when trained—help deliver flu vaccinations. This is a full‑time, permanent Band 3 role (up to £33,344 depending on experience), Monday to Friday during the working day, and will require travel across Plymouth, often at short notice, so a valid UK driving licence and mobility are essential. The post works autonomously within defined competencies and includes mandatory DBS checks and training.
