Play a pivotal role in Wiltshire Council’s Hospital In‑Reach team based at Salisbury District Hospital (SP2), delivering timely social work interventions that prevent unnecessary admissions and speed safe discharges. As an Experienced Social Worker you will carry out proportionate assessments, mental capacity work, and person‑centred planning while representing Adult Social Care on the Care Transfer Hub and weekend calls.
