Based in Peterborough (PE7) this permanent Assistant Care Co‑ordinator role supports patient flow across the county’s Intermediate Care Service. Working full time (37.5 hrs) at Band 4 (£27,485–£30,162 pro rata), you’ll work flexible early and late shifts within core service hours (07:00–22:00), covering weekends and bank holidays. Day‑to‑day you’ll receive and triage referrals, organise and monitor care packages, maintain accurate databases and records, liaise with health, social care and third‑sector partners, and occasionally provide face‑to‑face care calls and staff supervision to keep patients safe at home.
This role suits organised, compassionate carers who want hands‑on community work combined with coordination and administrative responsibility. A good standard of education (GCSEs or equivalent), IT skills, and NVQ3 (or willingness to train) are desirable; you must hold a full UK driving licence and use your own vehicle to meet strict time deadlines.
