of the criminal justice system to help to engage some of the most vulnerable service users as they leave prison.Be involved in the daily support of children and/or adults, including those from hard-to-reach communities to engage and navigate through the Criminal Justice System.The role is to support the service user in accessing support to

the full scope of identified health needs, including substance misuse (alcohol and drugs), mental and physical health, homelessness, learning disabilities, exploitation, safeguarding and social care issues.Reconnect is a care after custody service that seeks to improve the continuity of care of people leaving prison. This role seeks to reduce inequalities and

health-related drivers of offending behaviours. Whilst not a clinical service, Reconnect offers liaison, advocacy, signposting, and support to facilitate engagement with community-based health and support services.

We will treat and support individuals to reduce risks and improve outcomes.
