Job summary
The North Lincolnshire South PCN has a fantastic opportunity for a Care Coordinator to join our multi-disciplinary team. The position can be either full-time or part-time.Successful applicants will be based at one of the practices within the PCN (multiple appointments available). The practices in the South PCN include: Ancora Medical Practice, Ashby Turn Primary Care Centre, Cambridge Avenue Medical Centre, Cedar Medical Practice, Kirton and Scotter Surgeries and West Common Lane Teaching Practice.Care coordinators provide extra time, capacity, and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will work closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers and ensuring that their changing needs are
