Discharge Care Coordinator inLondon inLondon PUBLISHED 10 OCT 2024

We are looking for an enthusiastic discharge care coordinator who will support and coordinate discharge in different specialties and locations to improve patient care and the patient experience within the Trust. The post will play a pivotal part in ensuring there is a smooth transfer of care from UCLH to the community services. This will also offer an exciting opportunity for someone with previous healthcare experience to build upon their knowledge and experience in this innovative role.

You will work under the supervision of the registered nursing team and/or clinical leads, but will work independently in facilitating a smooth and timely discharge process for patients. This will include supporting the patient discharges from wards to create new capacity but ensuring that the continuity of care is maintained. To be successful in the role, the applicants must have a keen interest in improving the patient experience of discharge planning and patient flow in the wards, and care about the continuity of the service between the acute and community setting.
