We’re looking for compassionate and caring Healthcare Support Workers with a passion to be a key part of the patient care journey here at Crisis Response South Manchester. Our Healthcare Support Workers take great pride in the essential work we do. That’s because we’re not just helping patients, we’re going the extra mile for them – day in, day out. Our team is committed to caring for our patients and families, delivering consistent, high quality, patient focused care.You’ll communicate in a sensitive and compassionate manner with patients, relatives, and other members of the multi-disciplinary team. The service runs 7 days a week, varying shift patterns are available and operates between 8:30 am and 22:00pm and includes Bank Holidays, Christmas and New Year.
This is an exciting time to be part of our integrated multi-disciplinary and multi-agency team providing rapid assessment and support to people in their own homes.
Our Crisis Response team operates as a clinically led multi-disciplinary team comprising of Advanced Clinical Practitioners, Clinical Lead, Nurses, Occupational therapists, Physiotherapists, Social Workers, and Pharmacists.
You will be a champion of integrated community care approach, a strong team player and an excellent communicator. You will be adaptable, committed, well organised and able to work well under pressure. You must be able to travel around various community locations delivering care.
Car Owner And Driver Is Essential
Working under the supervision of ACPs, Nurses and therapists and within Trust guidelines & protocols, you’ll assist in delivering the excellent patient care you would expect for yourself and your loved ones.
Your duties will involve supporting patients with personal care needs, and will involve traveling to patients’ homes from various locations.
Following on the job training & assessment, you’ll learn how to undertake a range of delegated, duties that include recording patient observations & changes to clinical conditions.
Previous community experience is preferred; however, training will be available.
