The Dementia & Older People's Mental Health Service in Cornwall Partnership NHS Foundation provides assessment and treatment to people living with dementia and adults with functional mental health difficulties and increased complexity associated with physical ill health and the physiology of ageing.

This post is for a Community Psychiatrist Nurse working with people who have a dementia illness and / or a functional mental health illness, the latter the age of 75 and over.

The post is based at Longreach House, Redruth, with the Cober DOPMH Team, visiting people at home, in care homes and supporting our community health and social care teams.

The team in which you would be based have the following disciplines: A Consultant Psychiatrist, a Team Manager, a Memory Assessment Nurse, fellow Band 6 CPN's, a Clinical Psychologist, Primary Care & Dementia Practitioner's, a Dementia Liaison Nurse, Health Care Assistants, and an Occupational Therapist.

You will be a qualified professional RMN and have experience of working with people with dementia and/or mental health disorders and be able to manage a caseload under the Care Programme Approach placing the person at the centre of care in a busy but rewarding role. The ability to work in partnership with carers/family members and services in the community is essential.

The post holder will provide assessment, treatment and care co-ordination to two broad client groups:

  • People living with dementia.
  • Frail adults living with a range of mental health needs with the increased complexity associated with physical ill health and the physiology of ageing

The post holder will work as a senior member of the team and will have the responsibility to work closely with a wide range of both statutory and non-statutory agencies in developing and delivering personalised care under the care programme approach.
