Main duties of the job

The applicant must have experience in the management of minor illness/injury and is undertaking a qualification at Level 7 such as the APACs course (Advanced, Physical and Consultation Skills module) or minor illness course. Experience of chronic disease management is also desirable for this post. The Practitioner will be part of the Acute Care Team that will assess patients for urgent care on the day which may take the form of seeing patients face to face or by video consultation. The applicant must already have/ currently undertaking the non-medical prescribing training.

Job responsibilities

  • Assess, plan, develop, implement, and evaluate treatment programmes that promote health and well-being
  • Consult with patients presenting for acute minor illness assessment, treatment, and diagnosis at point of first contact within the ACT setting
  • Assess, plan, implement, evaluate, and review individual treatment plans for patients with a known long-term condition
  • Each individual will have a half day within their working week for their own speciality, to be decided with the individual at time of employment which will be beneficial to the needs of the patient population.
