To provide Health and social care cover for the Emergency Duty Service which is a generic social care team. The role includes coordinating and contributing to referrals in relation to Child safeguarding, Adult social care intervention, Emergency housing applications and Approved mental health contacts.
To provide and contribute to holistic needs led assessment and work with qualified social work staff to coordinate short term crisis interventions.
Children social care
To receive and record child safeguarding referrals and emergency update calls in relation to open child care cases.
To record and follow up contacts in relation to missing young people.
To act as an appropriate adult for young people in custody.
To contribute to welfare visits and assessments in relation to child safeguarding matters.
To support with the transport of children and young people in emergency situations.
Adult social care
To receive and record adult social care and adult safeguarding referrals.
To develop emergency support plans for adults with care and support needs which may include identifying and sourcing appropriate residential placements.
To act as an appropriate adult for vulnerable adults in custody.
To coordinate emergency health and social care interventions.
Housing
To receive and record referrals for those who are currently homeless or at risk of homelessness.
To contribute to the assessment of emergency housing need.
To locate and facilitate emergency accommodation to those in need.
Approved mental health
To received and record contact in relation to requests for mental health assessments.
To support and signpost callers to appropriate mental health services.
To act as appropriate adults for people with diagnosed mental health disorders who are in custody.
The Emergency Duty Service is a small specialist team of highly qualified professionals that work together in order to fulfil the local authorities statutory responsibilities out of hours. The nature of the work requires high levels of autonomy and ability to work flexibly under high levels of pressure.
