Job summary
To complement the existing Multi-Disciplinary Team (MDT) in ensuring the provision of MDT meetings facilitation within the localities, assisting them with delivering effective coordinated care, thus maximizing the health and quality of life of the patient/client and improving the quality and efficiency of health/social care.PLEASE NOTE, WE MAY CLOSE THIS VACANCY EARLY IF WE RECEIVE HIGH NUMBERS OF APPLICATIONS.
Main duties of the job
Working in conjunction with the Place Based locality model to co-ordinate ongoing patient/client case management.Documenting and monitoring all aspects of patient/client care coordination and service delivery.Collaborating and communicating with partners and wider health care teams to ensure the provision of appropriate services at the right time and in the right place.To organise and administer the MDT meetings including presenting information, taking and distributing minutes to minimise the demands upon the GP practice and multidisciplinary team.To attend Hospital and Specialist MDT meetings if required in support of patient identification and to improve care planning.
