We are looking for an

d Care Navigator - Receptionist to join our hard-working team. We are a smallfriendly surgery based in Portsmouth. welook after approximately 9750 patients. If you can work under pressure to highstandards and have excellent interpersonal and communication skills, we wouldlike to hear from you!The successful candidates will demonstrateexcellent communicate skills across multiple methods of communication, be ableto work under pressure, be flexible, reliable, and empathetic to our serviceusers needs and ideally have previous NHS

Main duties of the job
Receive, assist, signpost patients in accessing the appropriate service or healthcare professionalOffer general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone, website or email service.Deal with all general enquiries including booking, cancelling, and amending appointmentsFacilitate effective communication between patients, members of the primary health care team, secondary care, and other associated healthcare agenciesEnsure urgent and home visit requests are recorded and acted upon appropriately to ensure timely care for patientsReceive and make telephone calls as requiredPromote online services to patientsAdvise patients of relevant charges for private servicesTake payment and issue receipts for any chargeable services, ensuring accurate record keeping and informing patients when a charge is likely for work undertaken.Ensure all messages and communications received are accurate and delivered promptly.Receive and disseminate incoming mail as appropriateEnsure any safeguarding report requests are passed to the appropriate clinician in a timely mannerOpen and close premises in line with procedure including activation/deactivation of alarm.Retrieve and file patients notes as appropriate, ensuring filing is completed in accordance with local policy
