Job description
This is a fixed term/secondment post until 31/03/2027 to meet the needs of the Service.

Are you a qualified Occupational Therapist?Are you interested in supporting older adults with mental health needs who are in hospital in West Wales?Would you like to join the Older Adult side of Mental Health Liaison Service in Hywel Dda University Health Board?
We are a small and friendly team focused on improving patient outcomes, building links between general and mental health, and reducing inappropriate variation and stigma where possible. Mental Health Liaison Service provides assessments and interventions for patients across the age range 18+
The successful applicant would be part of a specialist multi-disciplinary team supporting patients within hospital settings who are experiencing co-existing mental health and physical health problems.
Assessing patients with a wide variety of mental and physical health comorbidities using a holistic approach, considering all aspects including social, emotional, and spiritual wellbeing within the hospital environment.
You would manage a caseload of people with complex needs in addition to providing clinical leadership, education and specialist advice for a range of other professionals, families, and carers.
Our main focus is in District General Hospitals, but the service also extends to Community Hospitals. One of our aims is to prevent avoidable admissions for people with mental health problems, alongside in-reach into the various hospitals. Mental Health Liaison Service supports professional development and encourages all staff to take an active role in service improvement.
This post is primarily for Older Adults, but there could be opportunities to add to the care plans of inpatients of working age too.
You will be based in Carmarthenshire at either Glangwili General Hospital or Prince Philip Hospital, and you will be expected to work in community hospital sites in Carmarthenshire if required.Mileage allowance is payable, pool cars and a lease car scheme is available.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Documenting assessments and interventions electronically in a timely and accurate manner.
Working flexibly and co-operatively as part of a team.Using initiative alongside clinical skills and experience to make decisions as appropriate within a supportive and experienced team.
Providing a positive image of people with complex physical and/or mental health needs; promoting independence and autonomy wherever possible for people living with dementia.
Compassionate in meeting the needs of vulnerable people and their families and carers; advocating for patients and carers as required.Using clinical reasoning/problem solving skills when liaising with other professionals/agencies in order to best meet the needs of patients.
Undertaking formal and informal education to increase skills and confidence around behaviour management and positive approaches to care for people living with dementia. Supporting staff to find the least restrictive approaches and interventions for patients experiencing delirium. Promoting the use of "This is Me" document to enable and empower staff to provide person centred care.
Understanding risk management and underpinning legislation, especially around Mental Capacity Act.
