Work nights delivering hands-on care and short-term support to people recently discharged from hospital and those needing an urgent community response across Derby City. As a Night Community Health & Social Care Assistant you’ll be based at Perth House Assessment Hub and visit people in their own homes between 21:45 and 07:30 on a rota that includes weekends and bank holidays. The role involves providing personal care, supporting rehabilitation goals, assisting clinicians with agreed care plans, working independently in the community and maintaining clear, professional records as part of a wider multi-disciplinary team. You will need a UK driving licence and access to a vehicle to meet travel requirements.

This permanent, part-time Band 3 role (salary £24,937–£26,598 pro rata) is aimed at experienced carers — minimum one year in health and social care — with GCSE Maths and English (C/4 or above) or equivalent and relevant NVQ3 or equivalent experience, IT skills and a willingness to travel locally; the post is subject to a DBS check.
