ModalityPartnership AWC Division has a part-time vacancy for 20 hours per week, fixed term for six months, for a Nurse Care Coordinator, to support with the E-falls project.The postholder will be a high performing employee who will work as part of amulti-disciplinary team across AWC Division. Main duties of the job
The role will ensure patient health and care planning is timely, efficient, andpatient-centred. This is achieved by bringing together all the informationabout a patients identified care and support needs and exploring options tomeet these within a single personalised care and support plan.The role is nota regular 9am-5pm job; the job requires innovation, flexibility and commitmentand the postholder will be required to work resourcefully as part of the teamto ensure tasks are completed.
