The Neighbourhood Nurse Lead at Lewisham and Greenwich NHS Trust (South Lewisham Health Centre, SE6 2SS) is a senior clinical role managing the neighbourhood community and district nursing team. As a Band 7 post (£47,810–£54,710 pa plus HCA), you will provide hands‑on clinical leadership, support patients with complex health and social needs, and work alongside GPs, social care and the voluntary sector to deliver seamless, person‑centred care. Key duties include proactive case management to prevent unplanned hospital admissions, assessing, diagnosing and prescribing in partnership with GPs, championing patient safety and clinical governance, using telehealth where appropriate, carrying out audits and data stratification, and maintaining NMC standards for records and practice. This is a permanent, full‑time role; applicants must hold current UK professional registration and have a full UK driving licence with daily access to a car. Closing date: 08 October 2025.

This vacancy is aimed at experienced first‑level registered nurses with degree‑level community or Advanced Nurse Practitioner qualifications, and either an independent prescribing qualification or a clear plan to obtain one.
