We are seeking two Occupational Therapists to join our Access and Information Team within Adult Social Care. This is a fantastic opportunity to make a real difference at the frontline of service delivery, supporting residents across Brent’s richly diverse communities to live independently and improve their quality of life. You will be part of a collaborative, people-centred team of professionals who are passionate about innovation, continuous learning and delivering high-quality, person-focused support. As part of Brent’s Access and Information Team, you will play a key role in supporting residents to navigate services and achieve independence. You will: - Undertake holistic assessments of residents’ needs to support independence, safety and wellbeing - Recommend and arrange suitable equipment, adaptations or reablement interventions - Provide advice and support to residents, carers and professionals, ensuring timely and accurate guidance - Work closely with Social Workers, Care Assessors, OT Assistants and other health and social care colleagues - Contribute to service improvement, innovation and person-centred practice within the team - Promote independence and prevent unnecessary hospital admissions through early intervention and collaborative problem-solving We’re looking for someone with: - A recognised Degree or Diploma in Occupational Therapy and current registration with the Health and Care Professions Council (HCPC) - Experience in Adult Social Care with a focus on assessment, equipment provision and enablement - A strong understanding of community-based occupational therapy and reablement approaches - The ability to work effectively in a fast-paced, multidisciplinary environment - Strong communication and relationship-building skills with residents, carers and professionals - A genuine passion for empowering people to live safely and independently within their community Experience in managing community equipment and delivering creative, person-centred solutions will be highly valued.
