Become an Older Adults Community Advanced Support Worker with Sheffield Health & Partnerships University NHS Foundation Trust, based at 7 Edmund Road, Sheffield S2. Two permanent full‑time roles (2 x 37.5 hours) are available on Band 3, salary circa £24,625–£25,674 pa pro rata. You’ll work within the Older Adults Home Treatment Team, providing short‑term, person‑centred support and holistic assessments for older people in mental health crisis — including dementia and functional illnesses such as depression, anxiety and psychosis. The team operates seven days a week (08:00–20:00) and you’ll visit people in their own homes and care homes, work alongside medics, nurses, occupational therapists and psychologists, help prevent hospital admissions and support timely discharges. This is hands‑on community work: carrying out care plans, observing and reporting changes in mental state, supporting medication self‑administration, offering domestic and social support, and responding in crisis situations. A full driving licence and access to a vehicle are essential.
