Worcestershire Acute Hospitals NHS Trust are looking for an experienced senior enhanced practitioner to join our new Hospital Outreach 24/7 service

To work closely alongside all speciality teams within our organisation, with a focus on caring for the sick, deteriorating and septic patient. This is a unique opportunity to work as part of a high performing multidisciplinary team working in the hospital

Job summary

This is a full time (37.5 hour) permanent cross site and full rotation across the 24-hour period and 7 days (if service requires)

The post holder will work as part of the Hospital Outreach 24/7 team responding quickly and effectively to sick, deteriorating and septic patients on the ward. They will work alongside multi-disciplinary teams using the Prevention, Identification, Escalation and Response (PIER) model to reduce physical deterioration.

Hospital Outreach 24/7 is a multi-professional, multispecialty approach to delivering care over a 24 hour period, with the aim of improving patient safety. We are looking for a dynamic, enthusiastic practitioner who is committed to the delivery of safe patient care in the ward and other areas outside of Critical Care, to join us and be a part of our developing Multi-disciplinary team.

We welcome applicants who are highly motivated with an acute clinical background who can demonstrate expertise in the assessment and management of acutely ill patients and meet the person specification. This post requires motivation, flexibility and initiative, the ability to prioritise workload in challenging situations and excellent communication and team skills. The applicant should be able to demonstrate the use of evidence based practice and be proactive in managing change.

About us

Job Purpose

The post holder will work as part of the Hospital Outreach 24/7 Team and will respond quickly and effectively to deteriorating patients on the wards. They will work alongside Multi-Disciplinary Teams (MDTs) using the Prevention, Identification, Escalation and Response (PIER) Model to reducing physical deterioration. They will also have responsibility for the daily management of a caseload of patients, in collaboration with the medical teams. The post holder will:

Work towards health promotion and prevention and comprehensively assess patients for risk factors and early signs of illness and deterioration.

Draw on a diverse range of knowledge in their decision-making to determine evidence-based therapeutic interventions, which will include prescribing medication where legally allowed and actively monitoring the effectiveness of therapeutic interventions.

Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, and provide direct support to patients and clients.

Work in partnership with patients and support them to manage and live their life.
