As a Patient Care Coordinator at Ramsbottom Group Practice (Ramsbottom, BL0), you will be the main point of contact for patients and healthcare professionals, coordinating appointments, referrals and follow‑up care to ensure continuity and timely access to services. The role includes supporting screening and vaccination programmes, linking patients to social care and mental health services (working with the Social Prescriber), maintaining accurate confidential records, helping on reception when needed and contributing to quality improvement and learning and development while ensuring CQC standards are met. The post is 32.5 hours per week (Monday to Friday) on a rolling 12‑month contract, pays £12.21 per hour and is subject to a DBS check.
