PCN Care Co‑Ordinator at Ancora Medical Practice, Ashby (North Lincolnshire South PCN) — a permanent role with full‑time, part‑time and flexible options available across six neighbouring practices. You will act as a single point of contact for a defined caseload, helping people and their carers prepare for and follow up clinical conversations, pulling together personalised care and support plans and coordinating navigation across primary care, social prescribing, health and wellbeing coaches and other community services. Day‑to‑day duties include case management, collation and coding of clinical information (EMIS/SystmOne/Ardens), booking appointments, supporting enhanced service delivery (including care homes), and contributing to multidisciplinary team work to ensure continuity of care for patients whose needs are changing.

This role suits someone with experience in a healthcare or caring setting (including unpaid caring), good IT and communication skills, and the ability to travel between sites (own transport required). You’ll receive training through the Personalised Care Institute if needed, work within a supportive PCN team, and undertake DBS clearance.
