Job summary
The SS9-North Primary Care Network (PCN) Care Coordinator willplay a key role in supporting the efficient operation of PCN services andimproving patient care delivery. This position involves coordinating clinicalrotas, maintaining accurate records and systems, supporting multidisciplinaryteams, and facilitating communication between PCN practices, communityservices, and partner organisations.The Care Coordinator will ensure that clinical andadministrative processes run smoothly, contributing to the effective deliveryof Enhanced Access and other Local / Directed Enhanced Services (LES/DES)Projects, Integrated Neighbourhood Teams (INT) collaboration, and generaloperational support for the PCN.

This role will provide vitaloperational, digital, and administrative support, helping to ensure the smooth running of PCN projects, programmes, and day-to-day activities.The postholder will work closely with PCN leadership, practice teams, and external partners to coordinate activities, track progress, and maintain clear communication across the network. They will also contribute to the implementation of digital tools and data systems that support improved patient care, workforce efficiency, and service delivery.
