PCN Care Coordinator at The Northwick Surgery (working with Sphere Primary Care Network) based in Pinner, HA5 — permanent, full-time role with a salary of £28,000 a year. You will work alongside clinicians across six GP practices to help people live healthy, independent lives by developing personalised care plans, supporting self‑management and prevention, and coordinating appointments and follow‑ups for long‑term conditions such as diabetes. The role blends patient contact and administration: maintaining records and registers, managing recalls and inboxes, handling referrals and clinic support, producing clear digital communications, and helping to develop and maintain the PCN’s website and digital platform. Experience in the NHS is essential; knowledge of EMIS and website development is desirable.
