in a Health, Social Care or Educational background workplace.The successful candidate will join our growing PCN workforce and support the delivery of care bringing together all the information about a patient's identified care and support needs and exploring options to meet these by identifying and signposting to appropriate clinicians.

Main duties of the job
We require a full time CareCoordinator to join our team to provide and maintain high standards of patientcare as well as administrative tasks. The successful candidate will enjoyworking as part of a dedicated team, with the added satisfaction of working foran organisation committed to the care and wellbeing of others.Specific areas of work include supportingGeneral Practices and the Primary Care Network to deliver the Local EnhancedServices including Improving Diabetes Care to our population living with Diabetes.You will also help with the development of Sphere Primary Care Network digitalplatform; help design, develop and manage the Sphere Primary Care Network Website.
