Job summary
Our Care Coordinators play an importantrole within the PCN to reduce health inequalities and support meetingour PCN and practice targets. They work closelywith practice and PCN staff to identify,engage with and coordinate personalised care and supportplanning for the most vulnerable peoplein our community, including the frail/elderly, patientswith dementia and their carers,patients diagnosed withcancer, care home residents and those with long-term health conditions.As well as being linked with individual practices they will worktogether as a team. This includes sharing learning and best practice bothwithin the team and across the PCN.Our Care Coordinators support Clinical Leads and theMulti-Disciplinary team in the organisation and facilitation of MDT meetingsincluding weekly Care homes meetings.They run reportsto proactively identifyeligible patients and work to increase uptakeof health checks,cancer screening, vaccinations and other services includingself-management services. Support with patient engagement, which includesensuring that information is accessible for all and having conversations withpatients and carers to increase understanding, alleviate concerns and increaseengagement and self-management.Find more information attached.
Main duties of the job
Coordinate multidisciplinary meetingsacross local care organisations including identifying patients in need of review and collating any information required tofacilitate their review prior to the meeting.Provide admin supportto multidisciplinary meetingsincluding taking minutes.Utilise GP Practiceclinical systems (SystmOne) and population healthdata to proactively identify relevantcohorts of patients to deliver personalised careSupport patients withinthese cohorts to access healthchecks and other health services including conducting blood pressure (BP)checks and phlebotomy (blood sample collection) where appropriate.Assist with the collection of clinical observations such as BP, pulse, oxygen saturation, and other basic healthindicators to support patientmonitoring.Find more information attached.
