Job description
Play a key role coordinating personalised care across York Road Primary Care Network (based in Leeds, LS14). As a PCN Clinical Care Coordinator you will work alongside GPs, ANPs, therapists, pharmacists and practice teams to identify, engage and support the most vulnerable in our population — frail and older people, those living with dementia and their carers, people with cancer, care home residents and those with long-term conditions. The role blends clinical and administrative duties: using SystmOne and population health data to run reports and find eligible patients, organising and supporting MDT and care-home meetings (including taking minutes), helping increase uptake of health checks, screenings and vaccinations, and carrying out basic clinical observations (BP, pulse, sats, peak flow) and phlebotomy where appropriate. You will have meaningful conversations with patients and carers to improve understanding, reduce barriers to care and encourage self-management.
This permanent, full-time post is ideal for people with experience in health, social care or caring roles — particularly women carers or those returning to work who want to turn hands-on caring experience into a clinical coordination career. The PCN supports a collaborative, learning-focused environment with opportunities to develop new skills, share best practice and influence local services for a population of around 33,000. Essential requirements include strong IT and communication skills, attention to detail, GCSEs (or equivalent) in English and Maths, a DBS check and access to transport; flexibility to work occasional extended hours is needed.
