Lead the development of a sustainable clinical pharmacy service across South Fulham PCN’s seven practices in Hammersmith & Fulham. This patient-facing PCN Clinical Pharmacist role combines clinical leadership with day-to-day prescribing work: you will carry out medication reviews, manage complex polypharmacy, support care-home and frailty cohorts, and focus on safer repeat prescribing and smoother transfers of care. The post supports national service specifications, QOF medicines targets and wider medicines management, with a clear remit for staff development and service integration. Appointments are a mix of remote and face-to-face clinics, with potential home visits.
This opportunity suits an experienced pharmacist with a degree, current GPhC registration and at least two years’ post-qualification clinical experience (hospital up to band 7 desirable); independent prescriber status and RPS membership are advantageous. The role is fixed-term (12 months), offers £50,000–£52,000pa (dependent on experience) and flexible working patterns (full-, part-time or flexible).
