Provide compassionate, person‑centred nursing in the community for Coastal PCN, delivering home visits and care‑home support across Brook Lane (SO31 7DQ), Lockswood (SO31 6DX) and Stubbington (PO14 2JP). This fixed‑term maternity cover (6 months) is part‑time (22.5–28 hours/week) and combines virtual and face‑to‑face clinics, long‑term condition reviews (QOF), vaccinations in care homes, advanced care planning and close working with GPs and multidisciplinary teams under the Enhanced Health in Care Homes (EHCH) framework.
This role suits an NMC/HCPC‑registered nurse experienced with older adults and frailty who has a full driving licence and access to a car. Coastal PCN offers a supportive, progressive environment with protected training, reflective practice and regular MDT collaboration that values your clinical judgment.
