This PCN Safeguarding Care Coordinator role, based with Nottingham City General Practice Alliance in NG16LD, combines hands‑on care coordination with dedicated safeguarding for children and vulnerable adults across three member practices (c.34,000 patients). You will support practice managers, safeguarding leads and clinicians, run and minute safeguarding MDTs, prepare statutory reports, maintain a vulnerable patient register and help practices meet CQC safeguarding expectations. The post is part‑time, fixed‑term (6 months), requires strong organisational skills, NVQ Level 3 or equivalent experience in health or care, and a DBS check; you must be available on the days the practices hold safeguarding MDT meetings and will also support the PCN’s on‑the‑day minor illness hub in the same building as one member practice.
