To achieve this, we carry out joint assessments, share our professional knowledge and work in partnership with our dedicated team administrators.As a PBS Practitioner, you will be an active team player and deliver on the PBS workstream with a view to delivering on high quality PBS strategies and formulations.In 2021, the service was nationally recognised and received a 'highly commended' accolade from the National Pharmacy Awards. In 2022 we reached the finals at the National Learning Disability and Autism Awards and later that year received the Trust's 'Outstanding Clinical Team of the year. More recently in 2025, we were recognised for 'Outstanding Patient
Experience
' from our Speech & language therapists and nationally, from the 'Beyond Words' charity for the 'Most Inspirational Outcome'.
Main duties of the job
You will work as an integral member of the Team whilst having responsibility for a designated caseload. The post holder will be expected to positively represent the needs and rights of people with learning disabilities, tackling inequalities and improving access to identified services in the statutory and 3rd sectorsYou will ensure that people receive a person-centered service, based on their assessed needs and work in partnership with generic health services, service users and carers to support the implementation of their formulated PBS strategies.You will undertake robust behaviour assessments & formulations/strategies and have a sound understanding of risk assessments and strategies.
