Key Responsibilities:As a GP Practice Nurse at Four Elms, you will:
- Conduct comprehensive patient assessments and develop individualized care plans
- Manage chronic conditions and provide preventive care
- Administer vaccinations and immunisations
- Perform wound care and assist in minor procedures
- Deliver health education and promote well-being in the community
- Work collaboratively with GPs, specialists, and other healthcare professionals to ensure holistic care
- Maintain accurate medical records and uphold the highest standards of confidentiality
Qualifications:
- Registered Nurse with current NMC licensure
- Experience in primary care or similar healthcare settings is an advantage
- Excellent communication skills and the ability to connect with patients across all ages and backgrounds
- Competence in medical documentation and basic IT skills
About us
To be responsible for the delivery of practice nursing services to the practice population, working collaboratively with the rest of the practice team.
The practice nurse is responsible for a number of clinical areas such as chronic disease management, vaccination and immunisation and general treatment room duties, as well as certain defined administrative duties.
The practice nurse role requires the ability to work both autonomously and within a team, good communication, assessment and risk-management skills in order to provide holistic, individualised care which promotes independence and self-care as much as possible.
