We are looking for an experienced Practice Nurse with independent prescribing to join our small, but dedicated team here at Stuart Road Surgery. The position is for 16 to 20 hours per week. Salary is dependant upon experience. Please state salary expectations when applying for the position.

To be fully proficient in Chronic disease management, vaccinations and immunisations, cervical screening, contraceptive reviews, wound care including compression, as well as other general treatment room duties.

To provide support to junior members of the nursing team including healthcare assistants and will direct their work to ensure that the Practices provide high quality care.

You will oversee infection control standards and cold chain compliancy.

To work within NMC Code of Conduct and to agreed Practice standards and protocols

Oversee recall systems for chronic diseases, cervical screening, immunisations, and other key areas as relevant to Practice Nursing

Delivery effective implementation of management of patients with Chronic Diseases as detailed by the Quality and Outcomes Framework, local quality schemes and national quality schemes

Support the implementation of evidence-based guidelines in conjunction with the wider team

Encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working

Oversee infection control and cold chain systems to ensure compliancy

Ensure that all nurse-related Care Quality Commission evidence table indicators are compliant, demonstrating evidence, review, and audit cycle

Clinical duties

Provide Chronic disease management for Diabetes, COPD, Asthma, and CHD/CVD

Perform cervical screening and promote breast awareness

Administer adult and paediatric injections

Provide adult and child immunisations ensuring full achievement of targets

Ensure safe storage, rotation, and disposal of vaccines and drugs

Undertake venepuncture and electrocardiography

Undertake wound care/ management / dressing techniques

Undertake leg ulcer management, dopplers and compression bandaging

Provide advice on lifestyle issues, assisting and supporting patients in changing their unhealthy behaviours and refer appropriately

Participate in the smoking cessation service

Deliver travel health care, including vaccinations and advice for patients

Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies, and be aware of local, referral procedures

Work with safeguarding teams where relevant

Ensure that current evidence-based approaches to patient care are standard practice

Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries

Undertake first aid and management of emergencies, e.g. burns, haemorrhages, hypo/hyperglycaemic emergencies/asthma attacks

Provides a comprehensive family planning service

Audit outcomes of care against standards and initiate change as necessary ensuring follow-up audit cycle

Maintain and update disease registers
