Based at the Halewood Centre, this hybrid role offers a dynamic opportunity to work with adults facing a range of physical, mental health, and social challenges. Key Responsibilities Manage a personal caseload with autonomy and efficiency Be on duty twice weekly, supporting urgent and complex cases Conduct assessments, safeguarding investigations, and Mental Capacity Act (MCA) evaluations Develop support plans and commission appropriate services Complete Continuing Healthcare (CHC) and Decision Support Tool (DST) assessments Collaborate with partner agencies including Rapid Response, Reablement, housing, health, and substance misuse services Promote independence through short-term interventions ✅ What You’ll Need Recent experience in adult social work (essential) SWE registration and a clean DBS Strong time management and caseload handling skills Car user with access to a vehicle (essential) Ability to identify and escalate risk appropriately Experience working with adults with learning disabilities, physical disabilities, and mental health needs Working Arrangements Full-time hours: 36/week Hybrid working: Minimum 2 days/week required on-site at Halewood Centre This is a fantastic opportunity to make a meaningful impact in a fast-paced, supportive team environment.
