This is an exciting opportunity to make a meaningful impact by supporting timely discharges and ensuring every patient receives the right level of care and continuity upon leaving in-patient rehabilitation. Key Responsibilities: Take full responsibility for all Neuro Rehabilitation and Discharge to Assess (D2A) cases. Manage and prioritise a varied caseload, ensuring no neuro rehab or D2A cases are left awaiting allocation. Work collaboratively with multi-disciplinary teams to facilitate safe and effective discharges for patients completing in-patient rehab. Complete robust assessments, support planning, and risk management in line with statutory duties and best practice. Maintain accurate and timely records in accordance with legislation and local policies. Essential Requirements: Qualified Social Worker with current SWE registration. Minimum 2 years’ post-qualification experience in Adult Social Care, ideally with a focus on hospital discharge, neuro rehab, or D2A pathways. Excellent understanding of the Care Act 2, Mental Capacity Act 2005, and hospital discharge processes. Ability to work effectively in fast-paced, multidisciplinary settings. Right to work in the UK.
