Maintain accurate and timely records in accordance with legislation and local policies. Essential Requirements: Qualified Social Worker with current SWE registration. Minimum 2 years’ post-qualification experience in Adult Social Care, ideally with a focus on hospital discharge, neuro rehab, or D2A pathways. Excellent understanding of the Care Act 2, Mental Capacity Act 2005, and hospital discharge processes. Ability to work effectively in fast-paced, multidisciplinary settings. Right to work in the UK.
