Lead a multidisciplinary Rehabilitation & Wellbeing team at Community Hospice, shaping palliative rehabilitation across community, inpatient, outpatient and hospital settings. You’ll lead occupational therapists, physiotherapists, wellbeing support workers and other allied health professionals to deliver holistic, person-centred care that helps people hold on to mobility, independence and dignity. The role combines hands‑on clinical practice, home visits and collaborative working with integrated neighbourhood teams, with service development responsibilities — using data, audits and stories to demonstrate impact, secure funding and keep standards high. You’ll also oversee equipment, governance and incident learning, support families through grief, and champion inclusion and clear, compassionate communication across the hospice and local health partners.
