The adults accepted into the service have been identified through the Transforming Care Agenda. The service will operate 24/7 using a trauma informed and case management model of care to provide support and opportunities for increased independence in the community for three adults. As a Relief Support Worker, you will be provided with support and direction from the support co-ordinator and service manager.
