Senior Health Care Support Worker #1111 with Douglas Macmillan Hospice (Dougie Mac), based in Stoke‑on‑Trent (ST3), provides hands‑on, community and hospice‑based support for people living with dementia and their families. Working closely with the Admiral Nurse team you will manage a clinical caseload, carry out home and hospice visits, deliver tailored non‑complex advice, emotional and enablement‑focused care, and make timely referrals to reduce crisis and improve wellbeing. Day‑to‑day duties include telephone and bereavement contact, observing and reporting changes in condition, keeping accurate electronic records, contributing to multidisciplinary care planning, and stepping in to provide basic physical care during crisis situations. The role requires flexibility to meet shift patterns, lone‑working awareness and the ability to travel independently across the locality.

This permanent, full‑time post offers a salary of £24,100 and a generous package including annual leave, contributory pension, funded study opportunities, a hospice‑paid healthcare cash plan and free on‑site parking.
