To identify when a safeguarding alert may need to be raised, ensuring the timely escalation of suspected abuse and self-neglect promptly To assess for and prescribe small pieces of equipment and telecare to assist individuals to achieve and maintain optimum independence Adhere to all health and safety instructions. Report to line manager any incidents, emergencies or accidents. To be accountable for delegated work and to ensure any work/instructions that are delegated to Reablement workers is communicated clearly. Maintain accurate written records, as well as statistics to ensure that service user details and details of the assessment/interventions delivered is recorded in the Trust database(s) To attend meetings as required providing verbal and written input as needed.in line with Trust policies. To attend and utilise supervision and appraisal to identify opportunities for development, new ways of working and reflection on practice; taking ownership of and responsibility for delivering against team and individual performance targets To attend training programmes required, achieving or maintaining competency in all required areas. Work in line with Trust policies and with National Care Standards and Care Quality Commission policies and procedures incorporating changing legislation and best practice Under the direction of the Reablement Co-ordinator to undertake observation visits to support Reablement Workers contributing to the identification of development needs. As a Health Care Assistant/ Support Worker you willbe responsible forparticipatingandassistingin the delivery of care / services to patients and service users as part of a Mental Health and Social Care Health Service. Health Care Assistant, Social CareWorkersand other Healthcare Support Workers employed by the Trustare required tofollow the guidance set out in Code of Conduct for Health Care Assistants/ Health Care Support Workers to provide safe and compassionate care of a high standard, and to challenge others who are not. You will receive direct supervision and partake in reflective practice / training on a regular basis and berequiredto meet the Care Certificate standards. All successful applicants will alsohave the opportunity todiscuss the prospect of undertaking a Level 3 Healthcare Support Worker Apprenticeship alongside their substantive post. This apprenticeship would include the Level 3 Diploma in Clinical Healthcare. This role is not eligible for sponsorship as per the Governments UK VISA and Immigration Rules and Regulations.
