Job description
Step into a senior coordination role within Rapid Response Torbay as a Band 4 Senior Referral Coordinator, based at St Edmunds in Torquay (TQ1). You will co-ordinate detailed referrals from health and social care professionals for people needing urgent support at home after a medical crisis, following hospital discharge or for end-of-life care. The role requires clear, confidential communication with multidisciplinary teams, care providers and the public, organising day-to-day and more complex caseloads, and providing direct line management and supervision of support workers to ensure Trust policies and quality standards are met. The service operates seven days a week, including bank holidays, and you may be required to work throughout the Torbay & South Devon area.
This permanent, full-time post (Agenda for Change Band 4) offers a salary of £27,485–£30,162 and genuine scope for development within Torbay and South Devon NHS Foundation Trust. Monthly supervision, annual appraisal and strong multidisciplinary links mean you’ll be supported to grow your leadership and administrative skills; NVQ3 (or equivalent), confident keyboard and IT ability, and previous health or social care experience are essential.
