Job summary
We are looking for a compassionate and supportive Senior Rehabilitation Assistant to join our friendly Community Neurological Rehabilitation Team (CNRT) in Southampton.As part of our CNRT multidisciplinary team, you will support us in providing specialist community therapy intervention to patients with a wide variety of neurological conditions.Your role will be integral in supporting patients to regain their independence in their own homes. You will provide rehabilitation intervention to a specialised caseload of patients, progressing and implementing programmes of care and reporting back to the MDT.You will be joining a friendly and supportive team of professionals who are keen to support you with CPD and in service training opportunities.Please note that a full driver's license and access to a car for community visits is essential for this role.For an informal visit to the service or conversation about this role, please contact Sarah Gibbs (Team Lead) on .We look forward to hearing from you!
Main duties of the job
Provide Senior Rehabilitation Assistant intervention to a specified caseload of patients with a Neurological diagnosis in the community setting.To provide comprehensive administrative support to the service.To effectively prioritise and manage own workload and oversee workload of team as necessary, whilst maintaining confidentiality and diplomacy at all times, both independently, or as part of a team.To be responsible for assessing patients, progressing and implementing programmes of care and reporting back to qualified staff. Providing planned rehabilitation care for clients who require regular support and intervention in the community, monitoring and modifying care plans as required.To work without direct supervision and show extensive use of initiative in prioritising and making decisions in relation to effectively supporting the smooth operation of the service.Support Spasticity clinics operation and preparation including the safe handling, retrieval and return of the botulinum and ITB medications.To carry out other duties as necessary to meet the changing needs of HIOWH NHS Trust, in accordance with management requirements and appropriate to banding.All duties will be carried out within the Trust's Information Governance, recording, and data sharing procedures and guidance.
