Responsibilities Provide high-quality care and support to clients in line with their individual support plans. Mentor and supervise junior support staff, providing guidance and training when needed. Develop and maintain accurate and comprehensive records of client interactions and support provided. Collaborate with healthcare professionals and families to ensure a holistic approach to client care. Facilitate engaging activities that promote independence, socialisation, and well-being for clients. Assess and monitor changes in clients' needs and adjust support plans accordingly. Participate in team meetings, contributing ideas and feedback to enhance service delivery. Requirements Minimum of 3 years of experience in a support worker role, preferably in a senior or leadership capacity. Relevant qualifications in Health and Social Care or a related field. Strong understanding of person-centered care principles and practices. Excellent communication and interpersonal skills, both written and verbal. Proven ability to work effectively within a team environment and to lead by example. Demonstrated organisational skills and attention to detail in record-keeping and reporting. Commitment to ongoing professional development and training in the care sector.
