Job description
As a Social Care Coordinator with NHS Jobs based in Northallerton (DL7), you will work full time on a permanent contract with a salary of £34,434–£38,220. The post sits within Community Social Care, the Locality Discharge Hub or the Review Team and involves asset- and strengths-based assessments, personalised support planning, determining eligibility against national standards, coordinating care packages and commissioning services. You will arrange equipment and adaptations, support reablement and direct payments, keep clear electronic records, act as a Safeguarding Adults Alerter and help enable timely, safe hospital discharges. The service operates seven days, so the role includes weekend duties on a rota and a mix of face-to-face and remote working depending on team needs.
