Job description
North Yorkshire Council is recruiting a Social Care Coordinator based in Northallerton (DL7) with the flexibility to be based at any of our county offices. This permanent, full‑time role (37 hours per week) offers a salary of £34,434 to £38,220 and hybrid working supported by modern IT. You will work within Urgent Care, Discharge Hub or Community Social Care teams to prevent avoidable hospital admissions and enable timely, safe discharges. The role involves carrying out asset‑ and strength‑based assessments, crafting personalised care and support plans, determining eligibility against national standards, arranging equipment and reablement, and progressing safeguarding actions when needed. You will work closely with social workers, therapists, nurses and health partners across hospital and community settings, with some weekend duties on a rota basis.
