Job summary
Calder & Ryburn PrimaryCare Network (PCN) consist of four GP practices and have a combined patientlist of over 43000 patients. Our practices are Bankfield Surgery, Brig RoydSurgery, Stainland Road Medical Centre and Station Road Surgery. We have grownas a collaborative PCN over the past five years and now have over 30 members ofstaff working across the PCN.Calder & Ryburn PCNare looking for a self-motivated individual to join our growing personalisedcare team. The team currently consists of care coordinators and socialprescribing link workers who all work in GP practices across the PCN supportingpatients from within the PCN footprint.This exciting role will beworking within Station Road Surgery and will join the fantastic practice teamand PCN colleagues.

Main duties of the job
Undertakework in line with PCN directed priorities.Facilitatingtransport options to enable attendance at surgery (efficient use of primarycare services), support groups and social schemes.InterrogatePractice records to identify patients where the biggest impact could be made,in line with PCN priorities.Bepractice based and work proactively to empower people to take control of their health andwellbeing, enhancing self-management, active care navigation by receptionistsand a greater understanding within practice and by patients of alternativeoptions and support outside of practice available to them.Establisha clear referral pathway from practices within the PCN and for self-referralfrom patients registered with practices in the PCN.Buildand maintain and understanding of existing social prescribing and navigationalservices available, together with their remit and referral criteria to supportcollaborative working.Workcollaboratively with all relevant local organisations, in line with directionfrom the PCN.Providepersonalised support to individuals, their families, and carers to take controlof their wellbeing, live independently and improve their health outcomes.Developtrusting relationships by giving people time and focus on what matters to me.Take a holistic approach, based on the persons priorities and the widerdeterminants of health.

Develop,manage, and update care plans within the GP clinical system used in the generalpractice (EMIS or SystmOne).Therole will require managing and prioritising your own caseload, in accordancewith the needs, priorities and any urgent support required by individuals onthe caseload. It is vital that you have a strong awareness and understanding ofwhen it is appropriate or necessary to refer people back to other healthprofessionals/agencies, when what the persons need is beyond the scope of thesocial prescribing link worker role e.g., when there is a physical or mentalhealth need requiring a qualified practitioner.Maintaina record of action and outcome that can be used for case studies and to keeppractices and the PCN informed.Promotingsocial prescribing, its role in self-management and active care navigation, andthe wider determinants of health with practices in the PCN and wider.Buildrelationships with key staff in GP practices within the local PCN, attendingrelevant meetings, becoming part of the wider network team, giving informationand feedback on social prescribing, how to access the service and makeappropriate referrals.Beproactive in developing strong links with all local statutory and third sectororganisations as appropriate, working in partnership with local agencies toraise awareness of social prescribing and how partnership working can reducepressure on statutory services, improve health outcomes and enable a holisticapproach to care.Beproactive in encouraging self-referrals and connecting with all localcommunities, particularly those communities that statutory agencies may findhard to reach.Meetpeople on a one-to-one basis.
