Main duties of the job
Wiltshire Support at Home offers short-term service for hospital discharges and individuals needing support at home, as part of a rapid response. Their crucial role includes preventing, reducing, and delaying the necessity for long-term care. The service aids those recently discharged from the hospital or under the care of the Rapid Response Service, providing short to mid-term homecare support to promote independence.We are seeking individuals with a genuine passion for helping others who are available on a rota basis between 7am to 10pm 365 days a year. Job responsibilities
Under the guidance and supervision of the Senior Support Worker (and from time to time other members of the service management team). Your role will be to support the customers with personal care, food preperation and daily living activities. Specific duties and responsibilities include:Work with customers to support them manage and maintain where possible their daily living skills, including washing, dressing, meal preparation, and personal careSafeguard the health, safety and wellbeing of customers as well as colleaguesTake part in staff development programmes, assuming responsibility for own professional development i.e. undertake all mandatory training relevant to the service and part of the training pathway for the role e.g. Care CertificateAssist customers with the use of mobility aids and assistive equipment where requiredAssist customers with moving and positioning, adhering to health & safety requirements as well as organisational policy and proceduresContribute to daily communications, accurate record keeping, recording actions, interventions and incidents in customers recordsParticipate in regular team meetings and customer review meetingsEnsure compliance with the Data Protection Act and maintain customer confidentiality and security of information at all timesCommunicate effectively with the immediate team and other health and social care professionals as appropriateParticipate in own supervisions, appraisals and objective settingAssist the team with general duties if required e.g. office filing, answering telephone calls etcWork as part of the team, offering a flexible approach to staff rosteringAbide by the principles and practice set out in the staff handbook
