Job description
Support Worker - Lived Experience with Practice Plus Group, based in Doncaster (DN4). Part-time (20 hours per week, Monday to Friday 9am–5pm), 18‑month fixed-term. Use your lived experience of the criminal justice system to engage people leaving prison and support vulnerable adults and children to access community health and support services. The role is non-clinical and centres on advocacy, liaison and signposting to address substance misuse, mental and physical health, homelessness, learning disabilities, exploitation, safeguarding and wider social care needs. A Disclosure and Barring Service (DBS) check is required and candidates must have the legal right to work in the UK.
