Job description
As a Band 4 Therapy Assistant Practitioner with The Dudley Group NHS Foundation Trust (Brierley Hill Health and Social Care Centre, Dudley, DY5), you will deliver the ‘home first’ approach by providing community rehabilitation and crisis response in people’s own homes. Working within a large multidisciplinary team you’ll offer rapid (up to 2‑hour) urgent care assessments and short‑term rehabilitation to help prevent unnecessary hospital admissions and A&E attendances. Day‑to‑day you’ll progress therapy programmes set by occupational therapists and physiotherapists, deliver exercise plans, provide equipment and enable personal and domestic daily living activities — working autonomously but with senior support. This permanent, full‑time role is AfC Band 4, salary £27,485–£30,162, and requires the physical ability to travel across the Dudley borough and carry equipment as part of timely community responses.
