The post will form part of a seven day service based within the Telford locality team.The service provides patients who are experiencing an acute change in their health, a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.The service offers an appropriate alternative to admission pathway, which aims to deliver a recommended and agreed medical, nursing & therapeutic treatment plan. The focus is supporting the safe management of people in their own home. Main duties of the job
You will be responsible to support the delivery of quality healthcare interventions to patients living in their own environment in the community, in order to meet their health and wellbeing needs, and to facilitate re-ablement support in order to maximise a patients functional independence at point of referral.You will support individuals and clinical teams to deliver high quality, clinically effective patient care through assessment, planning, implementation and evaluation. Along the way, you'll build strong relationships with a wide range of healthcare professionals and patients.Operating hours of the service will be 08.00-00.00, 7 days a week.
